CPPQ – Community Pharmacy Patient Questionnaire - Patient Survey

All pharmacies are required to conduct an annual community pharmacy patient questionnaire (CPPQ), also referred to as the Patient Satisfaction Questionnaire or Patient Survey. The questionnaire allows patients to provide valuable feedback to community pharmacies on the services they provide.

At Ebrington Pharmacy, we always try to provide you with the best possible service. We are asking NHS patients to complete a short and anonymous questionnaire about the service we provide.

Online Patient Questionnaire

You also have the option to download the forms and complete the questionnaire manually. Click here to download the Questionnaire in paper form. Once downloaded and completed, you can post the form to us or bring it in person.

We have Patient Questionnaire forms available in our pharmacy; please ask a member of the staff.

We will use the results of this survey to improve the services we provide at Ebrington Pharmacy.

Thank you for your ongoing support and valuable feedback.

HOW TO MAKE A COMPLAINT

At Ebrington Pharmacy, we welcome your comments and suggestions.

We aim to give you the highest standards of customer care and service. Should you have any complaints about our services or products please speak to our pharmacist. We operate a formal complaints procedure which is part of the NHS system for dealing with complaints. If you need any information, wish to leave a comment or suggestion, need to report an incident or lodge complaint about this website, our staff or any of our products or services, please contact us as soon as possible via the Contact Page on this website; alternatively, you can write to us at Ebrington Pharmacy, 61A Ebrington Street, Plymouth, PL4 9AA. Additionally, you can get advice from the local Patient Advice and Liaison Service (PALS). PALS is not part of the complaints procedure itself, but they might be able to resolve your concerns informally or can tell you more about the complaints procedure and independent complaint advocacy services.

Please visit the NHS Website for more information about how to make a complaint on an NHS Service received from us or from any other NHS organization.
